The team at UDCare offers Support Coordination basing on each participant's plan, helping you achieve your goals through providing information leading to proper informed decisions that meet your needs, We have caring Community Nurses that involves highly skilled Registered Nurses , Endorsed enrolled nurses and Support Workers offering quality care in the comfort of your home either ongoing care or post hospital discharge short-term care.To see how we can work together, reach out to us today.
How We're Different
UDCare is comprised of community nurses that are well motivated with caring people in their homes for many years, and that is why these registered nurses came together and became a registered NDIS provider in 2017 as a means to expand our capacity to help others: UDCare is my way of giving back to people what we have in us like you. We are unique from others that we extend our care in the absence of a Support worker, we do not talk about money, but we talk about quality care, involving our participants and carers in all the supports we render helping them achieve their goals with frequent assessments done . What We Offer
Offering a wide range of services for children to 65yr olds, our supports include:
- Support Coordination
- Assistance with community access
- Assistance with daily living
- Assistance with life stage transition
- Community Nursing offering complex and non complex supports
- Domestic support
- Transport assistance
We also help with complex needs including:
- Medication administration and management
- Colostomy and catheter care
- Wound management
- Continence aids assessments and recommendations
- In-house training
- Assisstance with chronic illness management (such as Diabetes, Epilepsy management, Palliative care etc.)
