Your Home Care offers a range of services for people of all ages that we can provide depending on your individual circumstances. All services are delivered in a caring, respectful and professional manner, with the aim to reduce the stress that can come with ill health.
Who we assist
We are a funded provider of Disability Services Qld under both the traditional model of delivery and we are a Host Provider under Your Life Your Choice. This means we have some flexibility in how we help you achieve your life goals and your quality of life.
We care for children and adults with physical, intellectual and mental health disabilities as well as veterans and the aged.
Like many others we are watching with anticipation to see what the implementation of the NDIS (National Disability Insurance Scheme) provides our existing and future clients, and how we can take the journey with each individual to live the best life that they can.
Providing the best care for your family
We can provide the best care for your family’s circumstances. We deliver our care with a high degree of sensitivity and empathy, which is very important in supporting families facing the difficulties of dealing with health problems. We have nurses and carers with the experience, commitment and expertise to deliver high quality care, and a range of services that can be tailored to individual needs and to people of all ages.
Personal care – Sometimes injury or illness can limit our ability to do even the basic daily activities. At Your Home Care we have nurses and carers who deliver high quality care assisting with hygiene, dressing, basic meal preparation, food shopping, running errands and even dropping and picking up the kids from school. We can provide support to those wishing to maintain their independence in their home for as long as possible, no matter what their needs are.
Support to respite and companion carers – We understand health problems can cause strain on the whole family and there is no shame in asking for help. It is important that support carers continue being wives, sons and friends rather than just a carer. They also need breaks to re-energise and at this time have the peace of mind that high level care is still being maintained.
Specialised nursing care – Our business model puts registered nurses out into the community. We have a great team of experienced nurses who can assist you with wound management, medication, stoma or catheter care, post-operative care, IV therapy and pain management.
Palliative care – We can provide support to the family and the terminally ill who wish to spend their last days in the comfort and love of home. Our nurses are very experienced in managing the difficulties of such emotional times.
Domestic Services – We can take away the worries of cleaning the house, maintaining the garden, getting the groceries, ironing, and running important errands. Just let us know what you need and we will take care of it for you.
Coordination of all care and equipment – Everyone’s circumstances are different, so we can advise you what equipment you will need and how to get it, or we can arrange it all for you.
