Hart to Hart started as a family business working in the NDIS in 2015. We service Victoria, and are also offering support in Sydney and Darwin.

Hart to Hart have a team of expert Support Coordinators who have been working in the NDIS landscape since the trial site stage. 

We offer a unique model of service delivery whereby we support our participants by considering their specific care needs first, and then seek services to meet those needs. 

This provides a model of true choice and control.

The NDIS services we provide are:

• Co-ordination of Support and Specialist Co-ordination of Supports
  We are experts in the industry with experience of Co-ordination of Support from the first day NDIS opened at Trial Site. 

  Hart to Hart is the service provider you were promised but have not received in this changing sector.

• Counselling Services
  We have skills in grief and loss, personal centred & solution focused therapy.  Group counselling also available.
   
• Group Activities
  These are offered in the Melbourne CBD for low supports needs funding.

  The activities we provide may include AFL games, Movies, Play board games, Museum and art gallery visits, walking groups and more.

We are a registered NDIS provider in Victoria and Northern Territory. We are also able to work with plan managers in Sydney, Central Coast & Lower Hunter Valley.
