Community Health Group, also known as CHG, is an NDIS Approved Provider. At CHG we understand that every person is unique, so we don’t take a one-size-fits-all approach. We pride ourselves in our ability to understand and cater to the needs of all individuals.

We work above and beyond, taking the extra steps needed to provide our best service. We focus on building a real connection with every person, their families and the community in which they live, to bring their goals to life. Prior to the engagement of our services, there is a thorough meeting with one of our team members to match each individual with an ideal candidate as a carer.

We focus on supporting, assisting and mentoring our participants throughout their lives to achieve goals and live a joyful life without any boundaries. We develop a plan tailored to suit each persons needs to fulfil their potential. We offer the opportunity to further empower and challenge each individual, whether it be learning new skills, enhancing current skills, trying new experiences or exploring new hobbies.

What we do:

We are a disability service provider, who empowers each person to reach their goals. We provide assistance in supporting each individual to live safely and comfortably in their home. Whatever the situation, we ensure each person is given support tailored to their needs.

Our services aim to assist each individual to live independently. We develop a care plan tailored specifically to each persons needs and lifestyle. The services listed above are covered under the scope of NDIS that we’ve been approved to provide.

We provide support throughout Sydney Metropolitan and rural NSW, with the prospects of expanding interstate.
