As one of the first NDIS providers in the Hunter region, Hunter Primary Care has been supporting participants since 2013. Our compassionate, multidisciplinary team helps you navigate the NDIS with confidence, delivering coordinated supports across Allied Health, Positive Behaviour Support, Psychology, Specialist Support Coordination and Group Programs. Our approach is person-centred and trauma-informed, focusing on your strengths and wellbeing. With over 30 years of experience in local health and community care, we provide therapeutic supports and assessments to help you reach your goals, build independence, maximise your funding and ensure everyone involved is working together to help you live the life you choose.
