Personal, respite and attendant care: Either at home or in the community, The Bridge Connects can support the needs of people with physical, sensory, cognitive and intellectual disabilities, including the frail and aged with various social and learning opportunities, as well as personal care requirements, enhancing your quality of life, networks and specific individual needs.

Service coordination: We can facilitate assessment of your support needs and help you to develop a support plan.

The Bridge Connects can also assist you with managing and coordinating external service providers ensuring your individual needs and wants are met.

General home care: We can assist with basic and routine household tasks such as cooking, cleaning and other light duties.

Overnight care: We can provide support staff for an overnight period, providing respite for carers and families, as well as yourself.

24-hour care: We can provide individual support workers to work any combination of hours please speak to the Individual Support Coordinator to discuss your needs.
